Superbill
I can provide a Superbill for reimbursement. A Superbill is a detailed receipt of services provided. You will submit that document to your insurance company for payment reimbursement. In order to ensure reimbursement, call your insurance company ahead of time.

Cancellation Policy
Cancellation / Rescheduling your appointment
Your appointment time is reserved specifically for you. Please provide 24 hours notice of appointment cancellations.
If less than 24 hours you will be charged the full session fee. Please note: Insurance companies do not cover cancellation fees therefore you will be responsible for the payment.
